Erexafil and your heart: see a doctor first

Erexafil is a dietary supplement labeled for men’s performance, libido and stamina support. It is not a treatment for erectile dysfunction, low testosterone or any other condition, and nine of its twelve ingredients sit in a blend with no individual amounts. That makes a doctor the first stop in each of these situations:

• Nitrates and heart medicine. If you take a nitrate for chest pain, or any heart medicine, ask your doctor first. ED medicines and nitrates must never be combined, as MedlinePlus explains, and a blend without amounts cannot be checked against them.
• Blood-pressure medicine. Ashwagandha in the blend may interact with blood-pressure and diabetes medicines, according to the NCCIH. See MedlinePlus on high blood pressure for why the plan belongs with your prescriber.
• Heart conditions. Chest pain, breathlessness or dizziness during exertion or sex needs urgent care; the NHLBI lists the warning signs of a heart attack. Call 911 if they come on suddenly.
• New erection problems. Trouble getting or keeping an erection that is new for you can be an early warning sign of heart disease, diabetes or a hormone problem. The NIDDK lists the conditions that can cause it, and MedlinePlus links the heart-disease connection. Get checked before you reach for a supplement.
• Hormones and DHEA. The blend contains DHEA, a hormone. Anyone with a hormone-sensitive condition such as prostate cancer, anyone on hormone therapy and anyone in drug-tested sport should not take it without a doctor’s OK. The NCCIH also advises people with hormone-sensitive prostate cancer to avoid ashwagandha.
• Blood thinners. Ginkgo in the blend may raise bleeding risk with anticoagulants.
• Vitamin D. One capsule carries 50 mcg, half the adult upper limit; see the NIH fact sheet before adding another vitamin D product.

If something unexpected happens after you start, stop taking it and tell your doctor; problems can also be reported to the FDA through MedWatch. Erection problems, a fall in drive and unexplained tiredness are medical questions before they are supplement questions: see a clinician first, especially if you take heart, blood-pressure, diabetes or hormone medicine.

DHEA, ginkgo and drug interactions

Erexafil contains DHEA, a hormone precursor that is prescription-only in several countries and prohibited by most anti-doping bodies, and ginkgo biloba, which can interact with anticoagulant medication. Nine of the twelve ingredients sit inside an undisclosed proprietary blend. If you take any medication, have a hormone-sensitive condition, or are subject to drug testing, speak with your doctor before using this product.
